"You can't run across difficult terrain, or if you can't see where you're going."

Does this mean that characters are prohibited from running if they are blindfolded?

What if they know the layout of an area, but someone turned off the lights a round ago?

The simplest answer is yes to both cases. Run is even faster than a double move action. Without visibility, the character is unable to run, and can only use a double move action, which means they can still move quite fast, but not as fast as a full out sprint.

If you wish to add more complexity, you may opt to require that characters who run without visibility (or other similar senses) will be required to make a Balance or Ride skill check to avoid tripping and falling regardless of the surface or any actual obstacles that might trip the character. It would provide a -10 penalty to any Balance, Flying, Ride, or Swim skill checks already required. Flying and Swim would not provoke any mandatory checks for lack of visibility because generally you cannot "trip and fall" while flying or swimming blind unless there is an obstacle already present.

Incorporeal creatures would not suffer these penalties as they can easily move through most objects regardless, although the GM is free to impose them if there are unseen warded or force obstacles in the way. Incorporeal creatures inside objects are considered blind anyway as long as their eyes are in solid matter.

Ultimately, the distinction here is between the ability to run while blind, and the ability to do so successfully without harm. The rules favour simplicity here rather than continuous checks, limiting characters to a double move when blind.
